The Clinical Inpatient Wound Care RN is a resource to physicians, nurses, patients, and families specializing in the care of patients with disorders of the gastrointestinal, genitourinary, and integumentary systems. Functions as an independent practitioner in the assessment, implementation, and revision of plans of care for patients with wounds, ostomies, fistulas, incontinence issues, and management of patients undergoing negative pressure wound therapy. Participates in the development of pressure injury prevention programs, process improvement initiatives, skin and wound care guidelines, patient and staff education, coordination of services, guidelines for specialty bed usage.

  • Provides consultation and specific clinical care to patients within the inpatient and outpatients wound care center needing instruction, management, nursing treatment plans for skin, wound, and ostomy care, and for incontinence management.
  • Documents assessments, plan of care, interventions, evaluation, outcomes and nursing orders on appropriate chart forms and the inpatient and ambulatory electronic medical record.
  • Promotes best practice by demonstrating a commitment to quality patient care and the profession of nursing.
  • Assesses need for patient education. Plans, implements and documents patient education and evaluates and documents patient comprehension and compliance.
  • Educates staff nurses in skin care, wound care, pressure wound staging and care, ostomy care, and encourages nurses to provide these measures independently within their scope of practice.
  • Identifies potential discharge needs and notifies Case Management or other services to provide follow up care and transition to the next level of care.
  • Recommends consultation with Wound Care specialist physician and or Wound Care Center for post discharge care when appropriate.
  • Uses appropriate resources for prevention, management and treatment of wounds, for supportive skin care, ostomies, and incontinence.
  • Analyzes patterns of wounds across programs and hospitals and identifies targeted PI activities to improve wound outcomes and mitigate pressure injury development.
  • Works with Clinical Education Department to establish competencies for clinicians regarding wound care through collaborate educational modules for wound care including patient and family teaching tools.
  • Provides input and feedback for wound and ostomy care policy and procedures.
  • Maintains and develops knowledge base and skills in the area of wound, ostomy, and continence therapy.
  • Maintains current professional knowledge and competence in the specialty areas.

Education:

Bachelor's degree in nursing (BSN). Wound Specialty Education from accredited program preferred (WOCN, WCC, CWCN, CWS, CWCA).

Experience:

Three years of experience working as a registered nurse in an acute care healthcare facility with minimum of 1-year wound care experience. Previous teaching experience preferred.

Other Credentials:

AHA BLS - Healthcare Provider, Registered Nurse - NJ or Registered Nurse - NLC multi-state.
