Due to our current Social Worker transferring to Assisted Living, we are seeking a full-time Social Worker for our nursing facility.  

Job duties for this position include, but are not limited to, the following:

  • Complete Quarterly, Annual and Significant Change Assessments (MDS) for residents and chart results.
  • Write progress notes for each resident at least monthly.
  • Complete social history for direct admissions to the Woodlands
  • Complete social history updates on all residents annually.
  • Social services care planning for all residents
  • Medicaid liaison- assist resident and/or family with Medicaid application as needed.
  • Contact family members of resident needs – clothing, etc…
  • Advise residents upon entering The Woodlands of their rights regarding Advance Directives.
  • Assist with discharge planning.
  • Must have basic understanding of Medicare and Long-Term Care insurance.
  • Educate and assist resident with appeal process from skilled care and initiate notice if non coverage
  • Complete and educate resident/family members regarding IPOST
  • Grievance officer
  • Complete PASRR for incoming residents
  • Assist resident/family member with initiating long term care insurance claims and submit monthly or as indicated
    • Funeral Coordinator:  Attend funeral for Woodlands residents or assign another staff member to attend.
  • Member of the following teams: Level of Care team, Interdisciplinary team, and Care Conference team
  • Social Worker services for Independent Living residents as needed.

Bachelor's degree in Social Work or similar degree with experience in health care or long-term care and retirement living preferred.  We offer a competitive wage and excellent benefits package.
