Pay Rate: Starting at $28.15/hour

We are seeking a friendly and caring Licensed Practical Nurse (LPN). The LPN, is responsible for
providing effective, personalized, and hospitable quality care and essential services to our resident
population, while achieving positive clinical outcomes and patient/family satisfaction. This essential
worker position is vital to the delivery of quality resident care. The LPN contributes to nursing
assessments and care planning, provides direct patient care, and supervises unlicensed staff.

The duties and responsibilities life of a Licensed Practical Nurse (LPN) include, but are not limited to:

  • Demonstrates dependable, regular, and consistent in-person attendance and punctuality.
  • Provides all the duties, responsibilities, and functions of a Licensed Practical Nurse that promote
    resident health, comfort, security, and wellbeing. Monitors own work to ensure quality. Meets
    productivity standards, completes work in timely manner, strives to increase productivity, and
    works at a quick but measured pace. Works with integrity and ethically, and upholds
    organizational mission and values. Completes other tasks as assigned by the Unit Manager, the
    RN Supervisor, or the Director of Nursing.
  • Makes daily compliance rounds to observe, examine, or interview residents
  • Performs physical evaluations and gather appropriate data of new admissions and current
    residents as indicated by change in condition or as required by regulation.
  • Administer medications, treatments, review labs, and complete skilled, change of condition,
    psychotropic and behavior charting
  • Assigns direct care staff; ensure consistent assignments, match resident needs, and monitor that
    care delivered is acceptable
  • Works as an interdisciplinary team member to develop and revise care plan goals and
    approaches to assist each resident to attain or maintain highest practicable physical, mental and
    psycho-social well-being.
  • Carries out shift commands to ensure continuity of care and report any significant issues to the
    nurse manager. Completes written 24-hour facility report.
  • Ensures compliance with all applicable Federal and State regulations, and internal policies and
    procedures.
  • Assists and monitors resident mealtimes.
  • Ensures residents are treated with dignity and respect.
  • Operates within the scope of practice defined by the Pennsylvania Professional Nursing Law, the
    Pennsylvania Nurse Licensure Compact Act, the Pennsylvania Practical Nurse Law, and other
    related Federal and State laws.

Required qualifications:

  • High school diploma or general education degree (GED).
  • Graduate of an accredited Practical Nursing program.
  • Unexpired license in good standing in the Commonwealth of Pennsylvania, or
    Nurse Licensure Compact (NLC) multistate license.
  • Unexpired CPR/BLS certificate.
  • Prior long-term care experience preferred, but not required.
  • Proficiency using Point-Click-Care or other EHR system.
  • Basic proficiency in MS Word, Excel, and general computer skills. Ability to navigate a computer
    using Windows operating system environment.
  • Excellent working knowledge of patient management, practical nurse practices and
    procedures, as well as the laws, regulations, and guidelines governing Licensed Practical Nurses.
  • Ability to read, understand, and interpret the English language. Ability to write and correspond in
    English using medical terminology when needed. Ability to communicate effectively in English.
  • Ability to do simple addition, subtraction, multiplication, and division using units of weight
    measurement, volume, and distance.
  • Possesses sound judgment, good reasoning skills, and ability to make independent decisions
    when circumstances warrant.
  • Possesses a customer service-oriented attitude. Possesses compassion, patience, tact, and a
    friendly disposition. Reacts well under pressure.
