Hiring Salary Range: $60,316 - $60,996

Sandhills Center is a Local Management Entity-Managed Care Organization (LME-MCO) serving people in eleven counties in central North Carolina. This position is responsible for providing oversight to a team providing I\DD care coordination services to members in the catchment area. The supervisor will work closely with team to develop work plans, goals, setting priorities, and ensure quality supports and consistent adherence to waiver requirements.

Minimum Requirements:

Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area or licensure as an RN, and 5 (five) years of experience providing care management, case management, or care coordination to complex members with I/DD or TBI.

OR

A master’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area or licensure as an RN, and 3 (three) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI.

Preferred Experience:

Work experience focused on working with Innovations Wavier preferred. A minimum of two (2) years’ experience supervising a team is preferred.

Must be a QDDP.
