Social Worker

Social Worker work for elderly healthcare center in San Gabriel Valley, CA for primarily Chinese speaking participants: review participants' health and financial data to assess needs and eligibility for healthcare, social and daily living programs; provide individual counseling and refer cases to healthcare professionals; recommend living and financial assistances; liaison with governmental and social agencies; implement systems for service and documentation.

Requires: Master in Aging Services Manager / Social Work with focus on Gerontology; six months' prior work experience as a social worker or intern in a senior living / healthcare facility; familiar with community policies and programs for senior living and healthcare, eligibility requirements and application process; familiar with participation assessment tools such as Motivational Interviewing, Mindfulness and Problem Solving Techniques. $33.75/hour, full time.
